تحلیل خطاهای پزشكی: یک مطالعه موردی

Abstract

زمینه و هدف: در بیمارستان‌های دولتی تهدیدات پنهان متعددی وجود دارند که در بروز خطاهای پزشکی مؤثرند. هدف از این مطالعه، بررسی فراوانی و نوع خطاهای پزشکی در بیمارستان طالقانی وابسته به دانشگاه علوم پزشکی شهید بهشتی، در سال 1393 بود.

مواد و روش‌ها: در این مطالعه توصیفی ـ تحلیلی، از فرم گزارش داوطلبانه خطا، طبق استانداردهای بیمارستان‌های دوستدار ایمنی بیمار، برای گردآوری داده‌ها استفاده شد. از میان 300 فرم توزیع شده، 162 فرم گزارش‌دهی خطا در طول یک‌سال جمع‌آوری شد. برای ورود داده‌ها از كد شناسه هر فرم استفاده شد، سپس از نرم‌افزار SPSS 19 جهت آزمون‌های کمی و برای تحلیل علی داده‌ها از نظرات 10 تن از متخصصان استفاده شد.

ملاحظات اخلاقی: تکمیل فرم گزارش خطاهای پزشكی به صورت داوطلبانه، بدون درج نام بیمار، فرد گزارش‌دهنده و بخشی كه در آن خطا اتفاق افتاده بود، انجام شد.

یافته‌ها: تعداد کل خطاهای دارویی در طی یک‌سال 158 مورد بود. فراوانی خطاها،‌ شامل خطای دارویی 38/6 درصد، خطا در فرآیند درمان 36/1 درصد و خطا در ثبت 25/3 درصد بود. آمار حوادث بدون عارضه 33/5 درصد، حوادث با عارضه 15/2 درصد و حوادث ناگوار 1/3 درصد خطاها را تشكیل می‌دادند. تحلیل خطاهای دارویی بر اساس شش اصل دارودهی شامل: بیمار اشتباه 32/14 درصد، داروی اشتباه 18/7 درصد، دستور اشتباه 15/3 درصد،‌ راه تجویز اشتباه 14/96 درصد، دوز اشتباه 11/9 درصد و زمان اشتباه 7% به دست آمد.

نتیجه‌گیری: علی‌رغم اطلاع‌رسانی و برگزاری جلسات آموزشی، موانعی بر سر راه گزارش‌دهی خطا در سیستم سلامت وجود دارد. از این رو آموزش اخلاق حرفه‌ای و رویکرد مدیریت سیستمی خطا توصیه می‌گردد.
