The Relationship between Ethical Reasoning and Emotional Labor in Nurses Working in Hospitals in Borujerd

Abstract
Background and Aim: Nurses, as the frontline individuals in the healthcare system, face various challenges in their work environment, including ethical challenges and in order to provide quality care, they must control their emotions in the workplace. This study was conducted to determine the relationship between ethical reasoning and emotional labor in nurses working in public hospitals in Borujerd in 2021.
Methods: The present study was conducted using a descriptive method with the participation of 160 nurses from public hospitals of Borujerd. Sampling was done using stratified random sampling after obtaining the ethical code and adhering to ethical standards. Data collection tools included a demographic information form and two questionnaires: The Grandey Emotional Labor Scale (α=0.88) and the Nursing Dilemma Test (α=0.82). Data analysis was performed using independent t-test, Chi-square test, One way ANOVA, Pearson correlation coefficient and linear regression model with SPSS 21 software. A significance level of less than 0.05 was considered.
Ethical Considerations: The principles of research ethics were observed: explaining the research objectives to the participants, obtaining informed consent, maintaining privacy, data confidentiality and providing the participants with the right to withdraw from the study at any stage of implementation.
Results: According to the findings, the level of ethical reasoning among nurses was 39.63±5.58, which was below average and undesirable based on the scoring of the tool used. The emotional labor status of nurses was 41.58±6.19, which was average and desirable based on the scoring of the tool used. There was no significant correlation between the level of ethical reasoning with emotional labor and its dimensions among nurses (P=0.628).
Conclusion: In this study, the ethical reasoning of nurses was not related to their emotional labor, but the increase in clinical reasoning and nurses' placement in difficult ethical situations had an impact on managing emotions in the nurses' work environment. These results can help nursing managers and planners by providing appropriate educational programs to prepare nurses for difficult ethical situations so that they can provide quality care and improve patients' health outcomes.
