ملاحظات اخلاقی افشای تشخیص به بیماران مبتلا به سرطان در ایران: یک مطالعه مروری سیستماتیک

چکیده
زمینه و هدف: اطلاع بیمار مبتلا به سرطان از بیماری خود، باعث درک بهتر بیماری و پیگیری دقیقتر درمان میشود. با توجه به ضرورت آگاهی بیمار سرطانی از تشخیص خود در روند درمان، موضوع چالشانگیز حالحاضر، بررسی جنبه های اخلاقی افشاسازی تشخیص به بیماران مبتلا به سرطان است. بنابراین هدف این مطالعه، مرور مطالعات داخلی انجامشده در مورد ملاحظات اخلاقی افشای تشخیص به بیماران سرطانی است.
مواد و روشها: جستجوی سیستماتیک در پایگاههای اطلاعاتی داخلی مگایران، علمنت، ایرانداک و نورمگ با واژگان کلیدی بیان تشخیص، افشای تشخیص، حساسیت اخلاقی، ملاحظات اخلاقی، بیماران سرطانی و بیماران مبتلا به سرطان به زبان فارسی صورت گرفت. هیچگونه محدودیت زمانی در انتخاب مقالات نبود. از 24 مقاله مورد بررسی، بر اساس ملاکهای ورود و خروج 14 مقاله خارج شدند و تحلیل نهایی در مورد 10 مقاله صورت پذیرفت.
یافتهها: نتایج این مطالعه نشان میدهد عمده پزشکان تمایل به افشای تشخیص دارند. آنها تمایل دارند تشخیص توسط خودشان بلافاصله و به صورت صریح و دریک جلسه و به طور تفصیلی بیان شود. تمایل پزشکان به افشا به متغیرهای مانند سن بیماران و تخصص آنها وابسته است. بیشتر بیمارانی که از تشخیص خود اطلاع دارند، در انتخاب درمان مشارکت فعالتری از خود نشان میدهند. بیماران ترجیح میدهند افشا، تدریجی و متناسب با درک آنها ارائه شود. بیماران در هنگام افشای تشخیص به حمایت از طرف خانواده، کارکنان بهداشتی، سازمانهای حمایتی و اطلاعرسانی درست احتیاج دارند.
ملاحظات اخلاقی: صداقت و امانتداری در گزارش متون، استناددهی به منابع و پرهیز از هرگونه سوگیری رعایت گردید.
نتیجهگیری: چگونگی بیان تشخیص و در نظرگرفتن مؤلفههای روانی و معنوی بیماران و خانوادههای آنها از یکسو و ویژگیهای روانشناختی پزشکان از سوی دیگر از چالشهای مهم در این فرایند در ایران هستند که احتیاج به بررسی بیشتر دارند.
