مقایسه اثربخشی گروه درمانی وجودی با گروه درمانی شناختی-رفتاری بر افسردگی بیماران تحت درمان نگهدارنده با متادون

چکیده
زمینه و هدف: اعتیاد به مواد مخدر، بیماري مزمنی است که در اغلب موارد با یک بیماري روان پزشکی دیگر مانند افسردگی می تواند همراه باشد. این پژوهش با هدف مقایسه اثربخشی گروه درمانی وجودی و گروه درمانی شناختی-رفتاری بر افسردگی افراد معتاد تحت درمان نگهدارنده با متادون انجام شد.
روش و مواد: روش پژوهش حاضر از نوع نیمه تجربی با طرح پیش آزمون، پس آزمون و مرحله پیگیری با گروه گواه بود. جامعه آماری مردان معتاد تحت درمان نگهدارنده متادون مراجعه کننده کلینیک های ترک اعتیاد و کاهش آسیب در قزوین در سال 1398 بودند که از بین آنها تعداد 90 نفر به صورت هدفمند انتخاب و در دو گروه آزمایش و یک گروه گواه قرار گرفتند. پس از جایگزینی تصادفی در سه گروه، گروه آزمایش اول تحت گروه درمانی وجودی و گروه آزمایش دوم تحت درمان
شناختی-رفتاری هر کدام در 10 جلسه 120 دقیقه اي به صورت هفتگی شرکت کردند اما گروه گواه، مداخله ای دریافت نکرد. هر سه گروه، قبل از مداخله، بعد از مداخله و پس از سه ماه، پرسشنامه افسردگی Beck را تکمیل کردند. داده ها با استفاده از آزمون اندازه گیری مکرر و نرم افزار SPSS-22 تحلیل شدند
یافته ها: در گروهی که مداخله شناختی-رفتاری صورت گرفت میانگین (انحراف معیار) افسردگی بیماران از (3/3) 5/44 در پیش آزمون به (1/3) 9/34 در پس آزمون و (5/3) 1/34 در مرحله پیگیری کاهش یافت (001/0>P). در گروهی که مداخله درمانی وجودی صورت گرفت میانگین (انحراف معیار) افسردگی بیماران از (2/3) 7/47 در پیش آزمون به (5/5) 1/37 در
پس آزمون و (4/5) 9/35 در مرحله پیگیری کاهش یافت (001/0>P). میانگین (انحراف معیار) افسردگی بیماران در سه مرحله پیش آزمون، پس آزمون و پیگیری تفاوتی نداشت. نتایج حاکی از آن بود که اندازه اثر درمان شناختی-رفتاری (53/0=Eta) بیش تر از اندازه اثر درمان وجودی (24/0=Eta) در کاهش افسردگی است که این مسأله نشان دهنده تأثیر بیشتر درمان شناختی-رفتاری در مقایسه با درمان وجودی در کاهش افسردگی بود.
نتیجه گیری: هر دو مداخله گروه درمانی شناختی-رفتاری و گروه درمانی وجودی باعث کاهش افسردگی بیماران تحت درمان نگهدارنده با متادون شدند، اما گروه درمانی شناختی-رفتاری مؤثرتر از گروه درمانی وجودی بود.
