پیامدهای فردی و اجتماعی شکایت از قصور پزشکی در میان پزشکان: یک مطالعه کیفی

چکیده
زمینه و هدف: امکان شکایت از قصور پزشکی اگر چه می تواند پزشکان را وادار به دقت در طبابت کند، اما ممکن است پیامدهای نامطلوب جسمی، روانی و اجتماعی برای پزشکان و بیماران به دنبال داشته باشد. هدف این مطالعه، تعیین پیامدهای منفی شکایت از قصور پزشکی بر پزشکان و نحوه طبابت آنهاست.
روش: این مطالعه کیفی به روش مصاحبه مسأله محور و به شیوه نمونه گیری هدفمند انجام شد. نمونه ها از میان پزشکانی که برای رسیدگی به پرونده قصور پزشکی به شورای حل اختلاف مستقر در سازمان نظام پزشکی تهران مراجعه کرده بودند، انتخاب شدند تعداد نمونه ها 15 نفر بود که با هر کدام از آنها پس از اعلام تمایل و رضایت آگاهانه، مصاحبه نیمه ساختار یافته انجام شد. داده ها با استفاده از کدگذاری و طبقه بندی تحلیل شدند.
یافته ها: پیامدهای روانی مانند استرس و اضطراب، پیامدهای شغلی مانند رفت و آمد مکرر به مراجع قضایی و غیبت از کار و پیامدهای اجتماعی مانند ضربه به اعتبار و شهرت پزشک و پیامدهای خانوادگی مانند نگرانی اعضا خانواده از جمله تبعات منفی شکایت از قصور پزشکی برای پزشک بودند. تغییر نحوه طبابت به سمت طب دفاعی، محافظه کاری و کاهش جرأت و جسارت پزشکان در طبابت کمتر از دیگر پیامدها بود. پرداخت خسارت هم به عنوان یکی از تبعات قصور پزشکی توسط پزشکان عمومی مطرح شد، اما پزشکان متخصص اشاره ای به آن نداشتند.
نتیجه گیری: مطالعه نشان داد که شکایت بیمار از پزشک، پیامدهای روانی و اجتماعی برای پزشکان در بر داشته است و با عکس العمل دفاعی، موجب پذیرش نشدن بیماران پرخطر و تحمیل هزینه های اضافی به بیماران شود. به اعتقاد مشارکت کنندگان در این مطالعه بررسی شکایت های بیماران در کمیسیون های کارشناسی و قبل از فراخوانده شدن پزشکان به مراجع قضایی می تواند در کاهش این مشکل کمک کننده باشد.
