الگوي سوختگي در کودکان پيش از دبستان

چکیده
سابقه و هدف: سوختگي يکي از شايعترين انواع آسيب در جهان است. مصدومين ناشي از سوختگي نسبت به ساير انواع تروما مدت بيشتري در بيمارستان بستري ميشوند. سوختگي منجر به معلوليت، ظاهر نابهنجار، مشکلات رواني و اختلال در رشد و تکامل کودکان ميگردد. افزايش آگاهي درباره چرايي و چگونگي وقوع سوختگي اولين گام درراه پيشگيري از سوختگي در کودکان است. هدف از اين تحقيق بررسي الگوي سوختگي در کودکان پيش از دبستان بود.
روش بررسي: در اين مطالعه مقطعي، اطلاعات مربوط به کودکان پيش از سن مدرسه که در طول يک سال از ابتداي مهر 1393، در بخش سوختگي بيمارستان امام رضا (ع) (مشهد، ايران) بستريشده بودند، با استفاده از سامانه اطلاعات بيمارستاني و با در نظر گرفتن معيارهاي اخلاقي بيانيه هلسينکي استخراج شد. اطلاعات مربوط به بيماراني که بيش از يکبار بستريشده بودند بهعنوان يک بيمار جمعبندي شد. تحليل دادهها با استفاده از نرمافزار SPSS نسخه 5/11 انجام گرديد.
يافتهها: در مدت يک سال انجام مطالعه، از 701 بيمار سوختگي بستريشده، 182 نفر (26%) کودکان زير 6 سال بودند. سوختگي در کودکان پسر شايعتر از دختربچهها بود (%3/59 در برابر %7/40). بيش از 90%سوختگيها در منزل اتفاق افتاده بود و آشپزخانه با 112 مورد سوختگي (5/61%)، از اين نظر نسبت بالاتري را داشت. همه سوختگيها از نوع حرارتي بودند و هيچ کودکي در اين گروه سني با سوختگي شيميايي يا الکتريکي بستري نگرديده بود. شايعترين ماده سوزاننده آب داغ (100 مورد، 9/54%) بود. کتري (49 مورد، 6/38%)، قابلمه (46 مورد، 2/36%) و شايعترين ظروف حاوي مايعات داغ بودند. در بيش از دوسوم اين حوادث (112 مورد، 67%) غفلت والدين يا عدم مراقبت کافي منجر به سوختگي شده بود و 24 مورد (2/13%) در حين پختن يا صرف غذا يا مايعات داغ توسط مادر، کودک دچار سوختگي شده بود. مرگومير در اين گروه سني در طول مدت بستري در بيمارستان، 85/3% و انفجار گاز شايعترين عامل مرگ بود.
نتيجهگيري: وقوع اکثريت وارد سوختگي در منزل و بهخصوص در آشپزخانه، لزوم آموزش والدين بهويژه مادران را در اين خصوص روشن مينمايد. کتابچههاي آموزشي درباره نکات ايمني و نحوه صحيح استفاده از وسايل آشپزخانه که با آب جوش سروکار دارند، ميتوانند کمککننده باشند.
