شناسايي و اولويت بندي عوامل موثر بر بروز خطاهاي انساني در بخش سلامت: مطالعه مروري منظم

چکیده
سابقه و هدف: با وجود تلاشهاي مداوم براي کاهش خطاهاي انساني در سيستم هاي مختلف، اشتباهات و رفتار نا ايمن علت اصلي وقوع حوادث در محيط کار مي باشد. مراکز درماني و بيمارستان ها يکي از مکان هايي است که رخداد خطاي انساني در آن به وفور امکان پذير است. بنابراين يک مرور سيستماتيک مطالعات گذشته، بهترين راه براي به اشتراک گذاشتن يافته هاي مفيد مطالعات و در دسترس قرار گرفتن گرايشات مطالعات آينده در زمينه خطاي انساني مي باشد.
روش بررسي: جستجوي الکترونيکي از پايگاه هاي اطلاعاتي پايگاه هاي اطلاعاتي مورد نظر صورت گرفت. اين مقالات از پايگاه هاي Pubmed, Scopus, Web of Science از ابتداي تاسيس آنها صورت گرفت. کليد واژه هاي مورد استفاده شامل (Performance shaping factors OR PSF) AND (human error OR human errors) AND (health care OR health system OR health section) بود.
يافته ها: پس از جستجو 6800 چکيده مقاله يافت شد. 5327 عنوان مقاله به دليل اينکه در راستاي اهداف مطالعه و معيارهاي ورود به مطالعه نبودند، حذف شدند. پس از بررسي 1473 چکيده به صورت مستقل توسط دو نفر محقق، 81 متن مقاله انتخاب و با مطالعه دقيق آنها در نهايت 30 مقاله در راستاي کامل موضوع مورد مطالعه انتخاب شدند.
نتيجه گيري: با تحليل نتايج مشخص شد سه عامل کمبود مهارت (1/12%)، نامناسب و ناکافي بودن تجهيزات (1/11%) و کمبود تجربه (1/11%) سه عامل اصلي اثر گذار در بروز خطاهاي انساني مي باشند. بيشترين و کمترين انواع خطاها به ترتيب به خطاي اجراي عمل (30%) و خطاي بازيابي (3/13%) بود.
