بررسي ارتباط افسردگي و اختلال شناختي با سقوط منجر به شکستگي در سالمندان

چکیده

 

 سابقه و هدف: سقوط یکی از مشکلات اصلی سلامت سالمندان است. هدف از انجام این مطالعه بررسی ارتباط افسردگی و اختلال شناختی با سقوط در سالمندان مراجعه‌کننده به بیمارستان‌های منتخب تهران بود.

روش بررسی: یک مطالعه‌ مورد- شاهدی که مبتنی بر بیمارستان‌های تهران در سال 94 انجام‌شده است. در این مطالعه 225 سالمند (81 مرد و 144 زن) ، شامل 75 مورد (27 مرد و 48 زن) و 150 شاهد (54 مرد و 96 زن) از میان سالمندان مراجعه‌کننده به 5 بیمارستان در مناطق: شمال، جنوب، شرق، غرب و مرکز تهران با یکدیگر مقایسه شدند. برای سنجش وضعیت شناختی از پرسشنامه استاندارد معاینه مختصر وضعیت شناختی و برای سنجش افسردگی از پرسشنامه افسردگی طب سالمندی استفاده شد. از آزمون‌های کای اسکوئر، من ویتنی، همبستگی و رگرسیون لجستیک برای تحلیل داده‌ها در نرم‌افزار STATA نسخه 12 استفاده شد.

یافته‌ها: نتایج مطالعه نشان داد که نسبت شانس سقوط در سالمندان افسرده 2/8 مرتبه بیشتر از سالمندان بدون افسردگی بود. همچنین نسبت شانس سقوط در سالمندان دارای نقص شناختی 7/6 مرتبه بیشتر از سالمندان بدون اختلال شناختی بود و همراهی این دو اختلال با یکدیگر، شانس سقوط، در مقایسه باوجود یک عامل به‌تنهایی را چند برابر می‌کرد.

نتیجه‌گیری: ارتباط مثبت و معناداری بین افسردگی و اختلال شناختی با سقوط وجود دارد. به‌منظور کاهش رخداد سقوط توصیه می‌شود سیاست‌گذاران، برنامه‌های آموزشی مناسبی را جهت پیشگیری از این عوامل خطر برای خانواده‌ها فراهم آورند.
