روغن‌های خوراکی غنی‌شده با آلفا-لینولنیک اسید از طریق کاتالیز آنزیمی لیپاز: یک مرور روایی بر مکانیسم‌های ضدالتهابی و پیشگیری از حوادث قلبی-عروقی به‌عنوان یک راهبرد بهداشت عمومی

چکیده

سابقه و هدف: بیماری‌های قلبی-عروقی سالانه حدود 8/19 میلیون مرگ ایجاد می‌کنند که 85٪ آن ناشی از سکته قلبی و مغزی است. التهاب سیستمیک و سندرم متابولیک از عوامل کلیدی بروز حوادث ایسکمیک هستند. با وجود پتانسیل محافظتی آلفا-لینولنیک اسید (ALA)، محدودیت‌های تغذیه‌ای و اکسیداتیو وجود دارد. هدف این مطالعه، تلفیق شواهد غنی‌سازی آنزیمی روغن‌های خوراکی با ALA برای پیشگیری از حوادث قلبی-عروقی است.

روش کار: جستجوی نظام‌مند در پایگاه‌های اطلاعاتی PubMed، SpringerLink و ScienceDirect (Elsevier) با کلیدواژه‌های مرتبط با ALA، لیپاز، غنی‌سازی آنزیمی، التهاب سیستمیک، سندرم متابولیک و پیشگیری از CVD انجام شد. جستجوی اولیه ۷۲۷ رکورد را شناسایی کرد که پس از حذف موارد تکراری و غربالگری سه‌مرحله‌ای (عنوان، چکیده و متن کامل)، ۶۸ مطالعه و سند واجد شرایط برای مرور نهایی انتخاب شدند.

یافته‌ها: غنی‌سازی آنزیمی با لیپازهای sn-1,3 اختصاصی و فناوری‌های تثبیت، روغن‌های غنی از ALA با پایداری اکسیداتیو بالا تولید می‌کند. ALA با مهار مسیر TLR4/MyD88/NF-κB و فعال‌سازی PPARα/PPARγ، TNF-α، IL-6 و CRP را کاهش می‌دهد. مصرف ≥۳ گرم در روز ALA لیپوپروتئین‌های آتروژنیک و Lp(a) را کاهش داده و از حوادث ایسکمیک پیشگیری می‌کند. روغن کاملینا مناسب‌ترین بستر و کانولا عملی‌ترین گزینه برای مداخلات جمعیتی است، هرچند افزایش خفیف LDL و پلی‌مورفیسم‌های FADS1/FADS2 باید مدنظر باشند.

نتیجه‌گیری: غنی‌سازی آنزیمی روغن‌های خوراکی با ALA می‌تواند راهبردی ایمن، غیرتهاجمی و قابل‌اجرا برای پیشگیری از حوادث قلبی‌عروقی در بهداشت عمومی باشد. بااین‌حال، تأیید اثربخشی و ایمنی آن نیازمند کارآزمایی‌های بالینی فاز ۳، توسعه سامانه‌های کپسوله‌سازی پایدار و تدوین چارچوب‌های نظارتی برای اجرای گسترده این رویکرد است.
