تبیین چالشهای مؤثر بر ایمنی و خطر مصدومیت در بیماران مبتلا به مولتیپل اسکلروزیس: یک مطالعه کیفی با رویکرد تحلیل مضمون

چکیده
سابقه و هدف: بیماران مبتلا به مولتیپل اسکلروزیس (ام اس) به دلیل اختلالات حرکتی، شناختی، روانی و مشکلات درمانی، در معرض خطر مصدومیت قرار دارند. این مطالعه با هدف تبیین چالشهای مؤثر بر ایمنی و خطر مصدومیت از دیدگاه بیماران انجام شد.
روش کار: این پژوهش کیفی با رویکرد تحلیل مضمون بر روی بیماران مبتلا به ام اس (18 تا 65 سال) مراجعهکننده به مراکز درمانی اردبیل انجام شد. نمونهگیری هدفمند تا اشباع نظری (11 مصاحبه) ادامه یافت. دادهها از طریق مصاحبههای نیمهساختاریافته گردآوری و با نرمافزار MAXQDA تحلیل شدند. ملاحظات اخلاقی رعایت شد.
یافتهها: تحلیل دادههای مصاحبهها نشان داد تجربه زیسته بیماران مبتلا به ام اس ماهیتی چندبعدی و پیچیده دارد. در کدگذاری باز، 34 واحد معنایی در شش دسته اصلی استخراج شد که بیشترین سهم مربوط به چالشهای روانی–احساسی (9 کد)، جسمی–حرکتی (8 کد)، درمان و دسترسی به خدمات سلامت (7 کد)، روابط بینفردی و مسائل اجتماعی (6 کد) و مشکلات مالی (2 کد) بود. در کدگذاری محوری، یافتهها در شش مقوله اصلی شامل مشکلات مالی، مشکلات جسمی–حرکتی، چالشهای احساسی، روابط بینفردی، چالشهای جامعهمحور و اقدامات تأمینی برای درمان سازماندهی شدند. این چالشها با افزایش خطر سقوط، محدودیت خودمراقبتی، کاهش پایبندی به درمان، اختلال عملکرد روزانه و کاهش ایمنی بیماران همراه بودند و نشان دادند که ام اس پدیدهای زیستی، روانی، اجتماعی و اقتصادی است که خطر آسیب و مصدومیت را افزایش میدهد.
نتیجهگیری: ام اس با چالشهای روانی، جسمی، اجتماعی و درمانی، ایمنی بیماران را تهدید میکند. ارتقای ایمنی مستلزم رویکردی جامع شامل حمایت روانی و اجتماعی، توانبخشی، آموزش خودمراقبتی، بهبود دسترسی به خدمات درمانی و پیشگیری از سقوط و مصدومیت است
