تحلیل ارتباط بین توزیع نهاده های بخش بهداشت با پیامدهای سلامتی در ایران با استفاده از ضریب جینی

چکیده
مقدمه: مقوله عدالت یکی از معیارهای مهم در توزیع منابع و دسترسی به خدمات بهداشتی درمانی میباشد. هدف این پژوهش تحلیل ارتباط بین توزیع نیروی انسانی بخش بهداشت با پیامدهای سلامتی در ایران (1387 – 1379) میباشد.
روش بررسی: این مطالعه توصیفی ـ تحلیلی و گذشتهنگر میباشد. برای محاسبه ضریب جینی هر کدام از متغیرها (پزشک، پرستار و تخت ثابت) و همچنین برای تعیین همبستگی هر کدام از متغیرها با شاخص سلامتی (مرگ و میر کودکان و مرگ و میر مادران) نرم افزار STATA مورد استفاده قرار گرفت.
یافتهها: ضریب جینی پرستاران در طی سالهای مورد مطالعه کاهش پیدا کرده در حالی که در ضریب جینی تخت کاهش چندانی دیده نمیشود و این ضریب برای نیروی پزشکان دارای افزایش بوده است. هر کدام از ضرایب جینی جداگانه برای تعیین همبستگی با مرگ و میر کودکان و مادران مورد تحلیل قرار گرفتند. در این بین تنها بین ضریب جینی پرستاران با مرگ و میر کودکان ارتباط معناداری مشاهده شد (p=0.009) و بین ضریب جینی پزشکان و مرگ و میر کودکان ارتباط معناداری به صورت عکس وجود داشته است(p=0.03).
نتیجهگیری: دلیل قاطعی برای بهبود شاخصهای بهداشتی مورد مطالعه در اثر افزایش عدالت در دسترسی به پزشک، پرستار و تخت بیمارستانی نمیتواند وجود داشته باشد و بهبود این شاخصها میتواند در نتیجه عواملی چون پیشرفت علم و تکنولوژی، افزایش مهارت نیروی انسانی و همچنین افزایش آگاهی مردم در زمینههای بهداشتی باشد.
