میزان تجویز آنتی بیوتیک پروفیلاکسی در زخم های تروماتیک ساده درمان شده در بخش اورژانس

چکیده
مقدمه: امروزه با توجه به پيشرفتهای ایجاد شده در مديريت زخم های تروماتيک، پیشگیری از عفونتهاي ثانویه به دنبال اين حوادث به عنوان يک چالش جدي مطرح شده است. استفاده از آنتي بيوتيک ها به منظور پروفيلاکسي جهت کاهش عفونت اگر چه نسبت به قبل بسیار کم رنگ تر شده است ولی همچنان به عنوان یکی از روش های کاهش عفونت مورد بحث می باشد. هدف از این مطالعه بررسی میزان مصرف و نحوه تجویز آنتی بیوتیک پروفیلاکسی در زخمهای تروماتیک درمان شده در بخش اورژانس و مقایسه آن با استانداردهای جهانی می باشد. روش کار: این مطالعه مقطعی در بخش اورژانس بیمارستان شهدای هفت تیر، تهران، ایران انجام گرفت. بیماران دچار زخم تروماتیک ساده به روش غیراحتمالی آسان انتخاب شدند. قبل از ترمیم زخم محقق زخم بیماران را از نظر شکل ظاهری، محل زخم، علت ایجاد زخم، طول و عمق زخم، مدت زمان ایجاد زخم و وجود یا عدم وجود آلودگی ظاهری مورد ارزیابی قرار داد و اندیکاسیون و یا عدم اندیکاسیون تجویز آنتی بیوتیک را بر اساس توصیه های رفرانسهای مورد تایید رشته طب اورژانس مشخص نمود. محقق مجددا بیماران را پس از ترمیم زخم در مورد دریافت و یا عدم دریافت آنتی بیوتیک پروفیلاکسی و مدت تجویز مورد پرسش قرار داد. داده ها پس ازجمع آوری با استفاده از نرم افزار آماری SPSS-21 مورد تجزیه و تحلیل قرار گرفتند. یافته ها: در مجموع 296 بیمار با میانگین سنی 74/14 ± 56/31 سال مورد بررسی قرار گرفتند که 6/81 درصد مرد بودند. جهت 268 نفر (5/90درصد) از بیماران مورد مطالعه آنتی بیوتیک تجویز شد. در حالی که تنها حدود 58 نفر (6/19درصد) از بیماران اندیکاسیون تجویز آنتی بیوتیک پروفیلاکسی را داشتند. تمام بیمارانی که اندیکاسیون دریافت آنتی بیوتیک داشتند، آنتی بیوتیک پروفیلاکسی دریافت نمودند. بر طبق نتایج این مطالعه جهت 210 نفر از بیماران (71 درصد) آنتی بیوتیک پروفیلاکسی بدون اندیکاسیون تجویز شد. بین میانگین مدت زمان تجویز آنتی بیوتیک پروفیلاکسی در مطالعه حاضر و مدت زمان استاندارد توصیه شده جهت زخم های تروماتیک ساده تفاوت معنی داری وجود داشت (0001/0 = p). زمان تجویز آنتی بیوتیک در مورد زخم های با آلودگی واضح (018/0 = p) و زخم های دارای اندیکاسیون تجویز آنتی بیوتیک میانگین زمان تجویز از 3 روز بیشتر بود (007/0 = p). جنسیت بیماران، نوع زخم و محل آناتومیک زخم تاثیر معنی داری بر میانگین مدت زمان تجویز آنتی بیوتیک با میزان توصیه شده نداشتند (05/0 < p). نتیجه گیری: بر اساس نتایج این مطالعه جهت 71 درصد از بیماران با زخم تروماتیک ساده آنتی بیوتیک پروفیلاکسی بدون وجود اندیکاسیون تجویز شد و میانگین مدت زمان تجویز آنتی بیوتیک پروفیلاکسی بیشتر از توصیه های استاندارد بود.- آنتی بیوتیک پروفیلاکسی؛ بخش اورژانس؛ عفونت زخم
