بررسی الگوی حساسیت دارویی 5 داروی ضد قارچی بر علیه گونه های آسپرژیلوس جدا شده از نمونه های بالینی بررسی حساسیت دارویی بر علیه گونه های آسپرژیلوس

چکیده
زمینه: ﻋﻔﻮﻧــﺖ آﺳﭙﺮژﯾﻠﻮزﯾﺲ ﻣﻬﺎﺟﻢ ﻫﻨﻮز ﻫﻢ ﯾﮑﯽ از ﻋﻠﺖ ﻫﺎی اﺻـﻠﯽ ﻣـــﺮگ و ﻣﯿـــﺮ در ﺑﯿﻤـــﺎران دارای اﯾﻤﻨـــﯽ ﻧﺎﮐﺎرآﻣـــﺪ می باشد. در مطالعات اخیر مقاومت گونه های آسپرژیلوس نسبت به داروهای ضد قارچی بخصوص آمفوتریسین ب و تری آزول ها گزارش شده است. الگوی حساسیت دارویی گونه های آسپرژیلوس جهت انتخاب داروی موثر و مصرف به موقع آن جهت بهبود وضعیت بیماران مبتلا بسیار ضروری است. مطالعه حاضر با هدف بررسی الگوی حساسیت دارویی داروی های ضد قارچی بر علیه گونه های آسپرژیلوس جدا شده از نمونه های بالینی انجام شد.
روش: ایزوله های آسپرژیلوس جدا شده از نمونه های بالینی بر اساس مشخصات ماکروسکوپی و میروسکوپی و همچنین تعیین توالی نوکلئوتیدی ناحیه ژنی ژن بتا توبولین شناسایی شدند. ارزیابی الگوی حساسیت داروهای آمفوتریسین ب، ایتراکونازول، وریکونازول، پوساکونازول و کاسپوفانجین بر علیه ایزوله های آسپرژیلوس بر اساس روش استاندارد میکرودایلوشن و مطابق آخرین نسخه از روش های تاییدی CLSI ، پروتکل M38-A2 برای قارچ های رشته ای انجام شد.
یافته ها: در طی مطالعه حاضر تعداد 163 نمونه بالینی از بیماران مشکوک به عفونت قارچی آسپرژیلوزیس از بیمارستان های شهر تهران و ساری مورد مطالعه بررسی شد. بر اساس شناسایی مولکولی، گونه های آسپرژیلوس فلاووس 42 (4/64 %)، آسپرژیلوس فومیگاتوس 12 (4/18 %)،آسپرژیلوس نایجر 8 (3/12 %) و آسپرژیلوس ترئوس 3 (6/4 %) شناسایی شدند. در بین داروهای مورد مطالعه، داروی کاسپوفانجین دارای بیشتری فعالیت ضد قارچی بر علیه ایزوله های آسپرژیلوس بوده و 100% ایزوله ها دارای MIC90 برابر با µg/mL063/0 بودند. در بین داروهای آزولی، داروی پوساکونازول دارای بیشترین فعالیت ضدقارچی بود و میزانMIC این دارو برای تمام ایزوله های مورد مطالعه برابر با ≤1 µg/mL بود. همچنین به ترتیب 5/0 % و 1 % ایزوله ها آسپرژیلوس فومیگاتوس نسبت به داروهای ایتراکونازول و وریکونازول مقاوم بودند.
نتیجه گیری: نتایج این مطالعه نشان داد که ایزوله های آسپرژیلوس جدا شده از نمونه های بالینی مورد مطالعه نسبت به داروهای کاسپوفانجین و پوساکونازول بسیار حساس می باشند. نتایج ارائه شده بصورت آزمایشگاهی در مطالعه نیاز به بررسی های بیشتر بخصوص در مطالعات مدل های حیوانی مبتلا به تظاهرات مختلف بیماری آسپرژیلوزیس مبتلا به این عامل دارد.
