مقایسة اثر کتامین با دکسمدتومیدین در بیدردی سوء مصرف کنندگان مخدر تحت جراحی نفرکتومی لاپاراسکوپی

چکیده
مقدمه:دکسمدتومیدین به عنوان یکی از ساختارهای دارویی در کنترل درد به کار رفته است و نتایج مختلفی از این دارو دیده شده است. کتامین اثر مناسبی در کاهش درد در افراد معتاد داشته است. دراین مطالعه اثر دوز پایین و بولوس کتامین با دکسمدتومیدین در کنترل درد افراد معتاد تحت نفرکتومی لاپاراسکوپیک مقایسه شده است. مواد و روشها:107بیمار معتاد که کاندید نفرکتومی لاپاراسکوپیکبودند انتخاب شدند. آنها بایستی سابقة مصرف روزانة مخدر را طی 6 هفته اخیر میداشتند. این بیماران به صورت تصادفی در دو گروه کتامین(K) و دکسمدتومیدین (D) قرار گرفتند.هر گروه برای کنترل درد حین عمل تحت انفوزیون رمیفنتانیل قرار گرفتند. میزان رمیفنتانیل حین عمل ( پیامد اولیه) و میزان مخدر مورد نیاز بعد از عمل(پیامد ثانویه) مورد مقایسه قرار گرفت. نتایج: میزان مخدر مصرفی به طور قابل ملاحظهای در دو گروه متفاوت بود. میزان رمیفنتانیل در گروهK496±34میکروگرم و در گروهD508±10میکروگرم و (001/0>P) بود و میزان مخدر مورد نیاز بعد از عمل در گروهK 1±4میلیگرم و در گروهD9±3میلیگرم با (001/0 P<) بود و عوارض قابل ملاحظهای در دو گروه دیده نشد. نتیجهگیری:کاهش قابل ملاحظهای در میزان مخدر مصرفی حین و بعد از عمل در گروه کتامین نسبت به گروه دکسمدتومیدین دیده شد.- کتامین، دکسمدتومیدین، کاهش مصرف مخدر، سوء مصرف کننده مخدر
