مقایسه بی دردی پس از عمل با مورفین و متادون عضلاني در بیماران دچار شکستگی تیبیا Comparison of intramuscular morphine and methadone on postoperative analgesia in patients with tibial fracture

چکیده
مقدمه و هدف: درد بعد از عمل جراحي براي بسياري از بيماران آزار دهنده است و ميتواند باعث بروز مشكلات متعددي گردد. اثر ضد دردي بسياري از مخدرها در حين و پس از عمل به صورت گسترده مورد مطالعه قرار گرفته است. در اين مطالعه بی دردی پس از عمل با مورفین و متادون عضلاني در بیماران دچار شکستگی تیبیا بررسي و مقايسه شد. مواد و روش كار: اين مطالعه ي مورد- شاهدی بر روي 60 بیمار کاندید دچار شکستگی تیبیا در سال 1392 در شهر زاهدان انجام شد. نمونه گیری با استفاده از بلوکهای تصادفی انجام شد و بیماران در دو گروه دریافت کننده متادون عضلانی و مرفین قرار گرفتند. در هر دو گروه میزان درد در زمان های مختلف و میزان نیاز به مسکن مورد بررسی قرار گرفت. داده ها با استفاده از آزمون های chi-squre و T-test و آنالیز شد. نتايج: میانگین سن بیماران 2/2±9/32 سال بود. 39 نفر (65%) از بیماران مرد و 21 نفر (35%) زن بودند. (05/0P) میانگین تعداد مصرف scuere در گروه دریافت کننده متادون 89/0±1/2 عدد برای هر بیمار و در گروه دریافت کننده مورفین 88/0±6/2 عدد برای هر نفر بود. (024/0=P) بحث و نتيجهگيري: مطالعه حاضر نشان داد که تجويز متادون بعد از عمل در بيماران دچار شکستگی تیبیا قادر به كاهش بيشتر درد بعد از عمل نسبت به مرفين است، بنابراين توصيه ميشود در این بيماران از متادون به عنوان مسکن بعد از عمل استفاده شود.- مورفین، متادون، بی دردی پس از عمل
