بررسی فراوانی عوارض بعد از بیهوشی در جراحی‌های شکستگی فمور در بخش مراقبت‌های بعد از بیهوشی بیمارستان امام خمینی ارومیه در سال ۱۴۰۳

Abstract

مقدمه: شکستگی استخوان فمور از جمله شکستگی‌های پرعارضه با مرگ‌ومیر قابل‌توجه است. مدیریت عوارض در بخش مراقبت های پس از بیهوشی (PACU) نقش مهمی در بهبود پیامد بیمار دارد. این مطالعه با هدف تعیین فراوانی عوارض بعد از بیهوشی در جراحی‌های شکستگی فمور در بخش مراقبت های ویژه انجام شد. مواد و روش‌ها: در این مطالعه مقطعی تحلیلی، تمامی بیماران تحت عمل جراحی شکستگی فمور در بیمارستان امام خمینی ارومیه طی سه ماهه اول سال ۱۴۰۳ به روش سرشماری وارد مطالعه شدند. اطلاعات دموگرافیک، مدت زمان اقامت در PACU و عوارض بروز یافته (درد، تهوع و استفراغ، مشکلات قلبی و تنفسی، لرز و بی‌قراری) جمع‌آوری و با استفاده از نرم‌افزار SPSS نسخه 26 و آزمون‌های آماری مناسب تحلیل شد. یافته‌ها: از ۱۰۰ بیمار مورد مطالعه، 74% مرد و 26% زن با میانگین سنی ۱۳/۳۸±۶۸/۱۹ سال بودند. میانگین مدت اقامت در PACU، ۴۹ دقیقه بود. شایع‌ترین عارضه گزارش‌شده درد (22%) بود. پس از آن، لرز و مشکلات قلبی هرکدام 10%، تهوع و استفراغ 9% و بی‌قراری و مشکلات تنفسی هرکدام 2% را تشکیل دادند. بین مدت زمان اقامت در PACU با سن بیماران همبستگی مثبت معنی‌دار وجود داشت (32/0 = r، 001/0=p). همچنین مدت اقامت در زنان به‌طور معنی‌داری بیشتر از مردان بود (02/0 = P). فراوانی تهوع و استفراغ در زنان به طور معنی‌داری بیشتر از مردان بود (9/26% در مقابل 7/2% و 001/0= P). فراوانی مشکلات قلبی (17% در مقابل 1/2% و 001/0 = P) و درد (9/36% در مقابل 1/2% و 001/0 > P) در بیماران تحت بیهوشی عمومی به طور معنی‌داری بیشتر از بیهوشی اسپاینال بود. ارتباط معنی‌داری بین سایر عوارض با جنس یا نوع بیهوشی و همچنین سن مشاهده نشد. نتیجه‌گیری: با توجه به شایع بودن درد به عنوان عمده‌ترین عارضه در PACU پس از جراحی شکستگی فمور، نظارت مداوم و اتخاذ رویکردهای مناسب بیهوشی و کنترل درد، به‌ویژه با توجه به سن، جنس و نوع بیهوشی بیمار، ضروری به نظر می‌رسد. خوشبختانه در این مطالعه عوارض تهدیدکننده حیات شایع نبود.
