بررسی تأثیر تجویز سولفات منیزیم وریدی بر اختلالات شناختی عصبی بعد از جراحی پیوند شریانکرونری

چکیده
مقدمه: نقص عملکرد شناختی سیستم عصبی در جراحی پیوند شریان کرونری یک عارضة شایع است. سولفات منیزیم یک آنتاگونیست غیر رقابتی گیرندة NMDA است که مصرف آن همراه با بهبود کوتاه مدت عملکرد نورولوژیک به ویژه در حافظة کوتاه مدت بوده است. هدف این مطالعه بررسی تأثير تجویز سولفات منيزيوم وریدی حين عمل جراحي پیوند شریان کرونری بر اختلالات شناختی عصبی پس از جراحي است. مواد و روشها: در یک مطالعة کارآزمایی بالینی دو سو کور 80 بیمار به طور تصادفی به 2 گروه تقسیم میشوند: دستة اول بیمارانی که 800 میلیگرم سولفات منیزیم 50% داخل وریدی طی 15 دقیقه حین القاء بیهوشی و به دنبال آن 3200 میلیگرم سولفات منیزیم طی 24 ساعت اول پس از عمل دریافت میکنند و دستة دوم بیمارانی که به همین روش و میزان، محلول نرمال سالین تزریقی دریافت میدارند. وضعیت شناختی همگی بیماران قبل و در روز 5 تا 7 بعداز عمل با استفاده از تست [1]MMSE تحت ارزیابی و مقایسه قرار میگیرد. یافتهها: بین دو گروه هیچ تفاوت معنیداری در مشخصات دموگرافیک یافت نشد. وضعيت شناختی بيماران در هر دو گروه بعد از جراحی نسبت به قبل از آن افت کرده است اما این کاهش در گروه سولفات منیزیم نسبت به گروه شاهد کمتر بوده است .(02/0p =) نتیجهگیری: تجویز منیزیم حین جراحی پیوند شریان کرونری میتواند سبب کاهش عوارض شناختی بعد از عمل بر اساس تستMMSE گردد.- سولفات منیزیم، جراحی پیوند شریان کرونری، اختلال عملکرد شناختی
