بررسی اثر دگز مدوتومیدین در درد حاد بعد از عمل جراحی ژنیکولوژی نازایی به روش لاپاروسکوپی

چکیده
مقدمه :مصرف داروی Dexmedetomidine (Dex) بهعنوان نوعی داروی آرامبخش و مسکن، کاربرد روزافزونی یافته است. چرا که علاوه بر خصوصیات آرامبخش و ضد درد، Cooperative sedation دارد و باعث دپرسیون تنفسی نمیشود. در این مطالعه قصد داریم تا اثربخشی درمانی و عوارض دارویی DEX را بر درد و آرامبخشی بعد از عمل بیمارانی که تحت عمل جراحی لاپاراسکوپی نازایی قرارگرفتهاند بسنجیم.مواد و روش ها:57 بیمار بهطورتصادفی در دو گروه (گروه D) ، 28نفر، دریافتکنندهی Dex ، µg/kg1حین القا به مدت 10 دقیقه وسپسµg/kg/h 0.5،دگز مدوتومیدین تا انتهای جراحی و (گروه C) ، 29 نفر شاهد، دریافت کنندهی نرمال سالین بهعنوان دارو نما تقسیم شدند. تمامی بیماران کاندید عمل جراحی لاپاراسکوپی نازایی بودند.نتایج :خصوصیات دموگرافیک بیماران نظیر سن و وزن در دو گروه تفاوت آماری معنیداری نداشت. ضربان قلب (HR) بیماران در گروه D بعد از القا و 15 دقیقه بعد از القا بهطور معنیداری نسبت به گروه کمتر بودp<0.008)و(p<0.04.MAP ، VASو آرامبخشی بیماران بعد از عمل دو گروه تفاوت معنی داری نداشتند.میزان مصرف مورفین پس از عمل در گروه D، 0.3±0.9و در گروه C، 1.0±1/2بود که بهطور معنیداری در گروه D کمتر از گروه C بود (.020(p<. میزان رضایتمندی بیماران در گروه D بهطور معنیداری بیشتر بود (007.0(p=.بحث و نتیجه گیری :مطالعه ما نشان داد که Dex دارویی مؤثر در جهت کاهش درد پس از عمل جراحی ژنیکولوژی لاپا روسکپی بوده و موجب افزایش رضایتمندی بیماران می گردد.در عین حال همودینامیک بیماران نسبتا" ثبات داشته ، گرچه ضربان قلب در زمانهایی کاهش داشته ولی خطرناک نبوده وبه آتروپین به خوبی پاسخ می داده است.آرامبخشی بعد از عمل در گروه دریافت کننده دگزمدتومیدین نیز تفاوت معنا داری با گروه شاهد نشد ومنجر به ترخیص دیر تر بیماران سرپایی نگردید.- لاپاروسکپی ،نازایی ،درد ، دگز مدتومیدین
