مقایسه بروز پنومونی لژیونلا پنوموفیلا در دو سیستم مرطوب کننده راه هوایی بیماران تحت تهویه مکانیکی: کارآزمایی بالینی تصادفی شده

چکیده
مقدمه: پنومونی مرتبط با تهویه مکانیکی شایعترین عفونت در بخش مراقبتهای ویژه است که میتواند باعث افزایش مرگ و میر بیماران گردد.لژیونلا پنوموفیلا یکی از عوامل مهم ایجاد کننده پنومونی در جامعه است. ما در این مطالعه به بررسی ارتباط بین نوع مرطوب کننده راه هوایی با میزان بروز پنومونی ناشی از لژیونلا پنوموفیلا در بیماران تحت تهویه مکانیکی پرداختیم.مواد و روشها: در مجموع 152 بیمار بستری انتوبه و متصل به ونتیلاتور در بخش مراقبت های ویژه که شرایط ورود به مطالعه را داشتند به صورت تصادفی تحت یک نوع از دو روش مرطوب کننده قرار دادیم. پس از تشخیص پنومونی دیررس ناشی از تهویه مکانیکی بیماران از نظر آنتی ژن ادراری لژیونلا پنوموفیلا بررسی شدند و نتایج حاصله مورد مقایسه قرار گرفتند. یافته ها: میزان بروز پنومونی ناشی از تهویه مکانیکی در دو گروه مرطوب ساز حرارتی ((HME و مبادله کننده دما و رطوبت (HH) تفاوت معنی داری از نظر آماری نداشت. همچنین میزان مرگ و میر در این دو گروه مشابه بود. میزان بروز لژیونلا پنوموفیلا در موارد پنومونی ناشی از تهویه مکانیکی پایین بوده و ارتباط معنی داری در بروز بیماری و مرگ و میر ناشی از آن بواسطه این عامل پاتوژن دیده نشد.بحث و نتیجه گیری: این دو نوع سیستم مرطوب کننده راه هوایی در بیماران تحت تهویه مکانیکی برای پیشگیری از پنومونی ناشی از لژیونلا پنوموفیلا برتریی نسبت به یکدیگر ندارند .- پنومونی ناشی از تهویه مکانیکی، مبادله کننده دما و رطوبت، مرطوب کننده حرارتی، لژیونلا پنوموفیلا
