میزان فراوانی پیامدهای نهایی در مراجعهکنندگان ترومایی ارجاعی توسط اورژانس پیش بیمارستانی با فرم سرپائی

چکیده
مقدمه: سیستم اورژانس پیش بیمارستانی دارای کدهای مشخص کشوری میباشد که دراعزام تکنسینهای اورژانس به مأموریت جهت گزارش کوتاه به مرکز هدایت عملیات استفاده میگیرد. در زمان حضور واحد تیم اورژانس پیش بیمارستانی در محل حادثه، در مواردی که نیاز به اعزام بیماران توسط آمبولانس نباشد، این گونه بیماران بعد از معاینه و تکمیل فرم 1029، از بیماران خواسته می شود تا خودشان راسا بصورت سرپائی به یکی از واحد های اورژانس بیمارستانهای شهر ، مراجعه نمایند. این مطالعه با هدف بررسی میزان فراوانی پیامدهای نهائی در این دسته از بیماران مبتلا به تروما ارجاعی با فرم 1029 بعد از پذیرش در واحد اورژانس بیمارستان شهید بهشتی قم، انجام شد.. روش ها : پژوهش حاضر یک مطالعه توصیفی مقطعی میباشد. جامعه پژوهش در این مطالعه را بیماران مراجعهکننده با فرم سرپایی اورژانس 115 به اورژانس مرکز بهشتی تشکیل داده اند. داده ها به صورت سر شماری تصادفی از بررسی پرونده بیماران دارای فرم اورژانس پیش بیمارستانی 115 در واحد مدارک پزشکی مرکز شهید بهشتی در طی سالهای 1392 الی 1395 جمعآوری شد. یافتهها: مطالعه نشان داد 13.41 درصد بیماران تحت عمل جراحی سرپائی قرا گرفتند، و 6.33 درصد برای بیشتر از 12 ساعت در اورژانس بستری شدند و 5 . 52 درصد در واحد مراقبت های ویژه بستری شدند. 10.78 درصد به اتاق عمل بیمارستان ارجاع شدند رابطه معنی داری بین متغیرهای جنس، سن و پیامد نهایی در بین بیماران ترومایی مراجعهکننده با فرم اورژانس دارای کد 1029 به بخش اورژانس مرکز شهید بهشتی وجود دارد (P=0.0001). اما بین متغیرهای زمان حضور اورژانس و منتظر ماندن در اورژانس و پیامد نهایی رابطه معنی داری مشاهده نشد (p=0.741 و p=0.552). بحث : این پژوهش با توجه به نتایج به دست آمده بیان میدارد که استفاده از کد 1029 برای 50% بیماران که نیاز به یک درمان سرپایی دارند و وضعیت حاد اورژانسی ندارند مناسب است ولی برای سایر بیماران(حدود 23% )بیماران که نیاز به مداخلات درمانی و اقدامات تشخیصی بیشتری داشتند مناسب نمی باشد.
