چکیده
مقدمه:هدف از مطالعه فوق مقایسه اثرات همودینامیک و ضد درد تزریق زیر جلدی دکسمدتومیدین ومارکائین 0/5 درصد دربیماران کاندید عمل جراحی هرنی و مدیریت درد بعد از عمل جراحی بود. روش اجرا:در این مطالعه کارازمایی بالینی دوسوکورتعداد 120بیمار که کاندید جراحی هرني در بیمارستان ولیعصر اراک بودند وارد مطالعه شدند.بیماران به سه گروه تقسیم شدند. در گروه اول مارکایین 0/5 درصد 5 میلی گرم ودکسمدتومیدین 1 میکروگرم در کیلوگرم ، در گروه دوم مارکایین 0/5 درصد 5 میلی گرم و درگروه سوم پلاسبو ، بصورت زیرجلدی تزریق شد . فشار خون ، ضربان قلب و درصد اشباع اکسیژن و میزان درد براساس مقياس آنالوگ بصری درد در ريكاوري ودر ساعتهاي 2 و 4 و6 و 12 و 24 ساعت پس از عمل و مقدار داروی مخدر مصرفی بعدازعمل و عوارضاحتمالي اندازه گيري شد. داده ها توسط نرم افزار spss 20 آنالیز گردید. نتایج: درد در گروه مارکائین و دکسمدتومیدن از دو گروه دیگر کمتر بود و در گروه پلاسبو بیشتر بود(0/0001 =p). کمترین میزان مصرف مخدر در گروه مارکائین به همراه دکسمدتومیدین دیده شد (0/0001 =p). نتیجه گیری: اضافه نمودن دکسمدتومیدین سبب کاهش درد و مخدر مصرفی بدون ایجاد عوارض جانبی در طی 24 ساعت بعد از عمل جراحی هرنی شد. کلمات کلیدی :تغییرات همودینامیک - تزریق زیر جلدی- دكسمدتوميدين- مارکائین 0/5 درصد Abstract Introduction: The study addressed the comparative hemodynamic and analgesic effects of subcutaneous dexmedetomidine versus marcaine 0.5% on herniorrhaphy scheduled patients, as well as postoperative pain management. Methods: This double-blind trial enrolled three groups of patients (N=120) scheduled for herniorrhaphy at Valiasr Hospital (Arak, Iran): (i) MAR-DEX group, receiving marcaine 0.5% (5 mg) + dexmedetomidine (1 mcg/kg), (ii) MAR group, marcaine 0.5% (5 mg), and (iii) PBO group, placebo, subcutaneously. We measured vital signs (BP/HR/SaO2), as well as pain scores (using the visual analog scale) at recovery and certain time points (1, 2, 4, 6, 12, 24 hrs post-op), and the overall opioid administered postoperatively and side effects. This was afterward followed by an SPSS (20) analysis. Results: Lower pain was revealed in the MAR-DEX group and higher one in the PBO group (p = 0.0001), whereas the lowest opioid use was observed in the MAR-DEX group (p = 0.0001). Conclusion: Adding dexmedetomidine had benefits of relieving pain and reducing opioid use without any side effects.
