شیوع انقباض عضلات ادداکتور متعاقب بلوک اوبتوراتور به روش معمول به همراه اسپاینال آنستزی در رزکسیون تودههای مثانه

Abstract
عمل جراحی تومور مثانه از طریق مجرا معمولاً با اسپاینال آنستزی مدیریت میگردد و انجام این روش گاهی بعلت تحریک مستقیم عضلات ادداکتور بخصوص در تودههای لترال مثانه باعث عوارضی چون پرفوراسیون میگردد. مواد و روشها تعداد 124 بیمار تحت یک مطالعه آیندهنگر توصیفی از نظر میزان شیوع اسپاسم عضلات ادداکتور و بروز عوارض از جمله پرفوراسیون مثانه قرار گرفتند. این مطالعه در بیماران مراجعه کننده به بیمارستان شهداء تجریش با توده مثانه نیازمند به عمل جراحی TURT انجام گرفت. نتایج در 124 بیمار مورد بررسی تحت اسپاینال آنستزی به همراه بلوک اوبتوراتور به روش معمول (Blind) 8 مورد دچار انقباض عضله ادداکتور شدند (3/6%) پرفوراسیون نیز در 1 نفر (9/1%) اتفاق افتاد. بحث میزان موفقیت بلوک اوبتوراتور به روش معمول (Blind) در این مطالعه 7/93% بود که در مطالعات دیگر بین 89 تا 100 درصد گزارش شده است. تجربه و مهارت متخصصین بیهوشی نقش عمدهای در میزان موفقیت دارد. استفاده از روشهای کمکی از جمله نرولوکیتور و سونوگرافی بر بهبود میزان موفقیت مؤثر است.- anesthesiology
