چکیده
مقدمه : ایجاد استرس های اکسیداتیو به دلیل تماس خون با سطوح مختلف مواد صناعی از چالش های تیم درمانی طی جراحی بای پس قلبی ریوی ،محسوب می شود. این مسئله با آزاد سازی عوامل التهابی همچون CRP، IL-6 و نوتروفیل ها سبب آسیب های بافتی به اندام های مختلف می گردد. با توجه به تمامی تلاشهای انجام شده در سالهای اخیر به منظور کاهش این آثار مخرب ، هنوز هیچ یک از روش های مرسوم نتوانسته اند بعنوان روشی کاملا مطمئن مطرح گردند. مواد آنتی اکسیدان همچون تیامین در دفاع از سلول ها در برابر رادیکال های آزاد اهمیت فراوانی دارند. در پژوهش حاضر تاثیر تیامین بعنوان ماده آنتی اکسیدان بر سطح لاکتات سرم در بیماران تحت عمل جراحی بای پس عروق کرونری مورد ارزیابی قرار گرفته است. مواد و روش : در این مطالعه 140 بیمار با رنج سنی بین 25 تا 65 سال و مبتلا به اختلال خفیف سیستولیک (EF=45-55%) که کاندیدای جراحی CABG بودند انتخاب و به دو گروه ( 70 نفره) کنترل و غربال (بیماران با دریافت تیامین) تقسیم شدند. تمام این بیماران جهت جراحی بای پس قلبی تحت پمپ قلبی-ریوی، به روش یکسانی بیهوش شده و سطح لاکتات سرمی آنها قبل ، حین و بترتیب 2، 4، 6، 12، 18و 24 ساعت پس از جراحی اندازه گیری شد.تمامی داده های بدست آمده طی پرسشنامه ای جمع آوری شده و با استفاده از نرم افزار آماری SPSS مورد ارزیابی قرار گرفتند. نتایج : بر اساس اطلاعات بدست آمده ، اختلاف معنی داری بین دو گروه مورد مطالعه،از نظر اطلاعات دموگرافیکی و بیماری زمینه ای مشاهده نشد. سطح لاکتات سرم در گروه تیامین نسبت به گروه کنترل بطور معنی داری کمتر بود ( بجز قبل و 2 ساعت پس از جراحی) (P<0.05). کراتینین سرم نیز درگروه تیامین ،در نمونه اخذ شده در 24 ساعت بعد از جراحی به صورت معنی داری کمتر از گروه کنترل گزارش گردید(0.14±1.54در مقابل 0.19 ±1.24 ، p: 0.001).همچنین نتایج حاصله نشان دادند که دوز مصرفی داروهای اینوتروپ ومدت زمان انتوباسیون بعدازجراحی در بیماران تحت درمان با تیامین نسبت به گروه کنترل، کمتر بوده (P=0.001) ولی مدت زمان بستری در بیمارستان در دوگروه مشابه بوده است. جمع بندی: نتیج نشان دادند که تجویز تیامین پیش از جراحی قلب ( در بیماران با اختلال خفیف سیستولیک بطن چپ) ضمن کاهش سطح لاکتات سرم (بعنوان مارکر پرفیوژن بافتی)، سبب بهبود عملکرد کلیه ها نیزمی گردد.- تیامین ؛ بای پس عروق کرونری؛اختلال سیستولیک؛لاکتات
