بررسی مقایسه ای تزریق متادون و سوفنتانیل وریدی بر میزان خونریزی در بیماران دارای اعتیاد به اپیوم تحت جراحی Bypass قلبی Comparative study of intravenous methadone and sufentanil effects on bleeding volume in opioid addicted patients undergoing cardiac bypass surgery

چکیده
مقدمه و هدف: هدف از انجام این مطالعه تعیین و مقایسه تزریق متادون و سوفنتانیل وریدی بر میزان خونریزی در بیماران دارای اعتیاد به اپیوم تحت جراحی Bypass قلبی در بیمارستان امام علی(ع) کرمانشاه بود. مواد و روشها: این مطالعه از نوع مطالعات تجربی است که به صورت کارآزمایی بالینی صورت گرفت.در این مطالعه در طی مدتی که نمونه های تعیین شده جمع آوری شوند تمام بیماران دارای اعتیاد به تریاک یاهرویین یاکدیین, مراجعه کننده به بیمارستن امام علی (ع) کرمانشاه که تحت جراحی bypass قلبی قرارگرفته اند وارد مطالعه می شوند وبصورت راندوم به دو گروه درمان بامتادون وسوفنتانیل وریدی تقسیم می شوند.بیمارانی که به صورت off pump تحت جراحی قرار گرفته باشند واردمطالعه خواهند شد.دراین مدت بیمارانی که معیارهای خروج ازطرح را نداشته باشند به صورت تصادفی به دو گروه درمان باتزریق متادون0.5mg/kgو ودرمان باسوفنتانیل3 میکروگرم به ازای وزن حین وپس ازجراحی ودر بخش مراقبت های ویژه, متادون بادوز0.5-2 mg وسوفنتانیل بادوز5-10میکروگرم تقسیم می شوند ودر هرگروه میزان خونریزی باتوجه به حجم درن ثبت می شود. و میزان خونریزی آن ها درطی مدت 24 ساعت شبانه روز در بخش مراقبت های ویژه سنجیده می شود. هم چنین اطلاعات مربوط به با استفاده از پرسشنامه ثبت گردیدو با نرم افزار SPSS تحلیل گردید. یافته ها: میانگین سنی در گروه بیماران که متادون را دریافت کرده بودند 84/8 ± 83/63 و در گروه بیماران که سوفنتانیل دریافت کرده بودند 11/8 ± 48/66 بود. در گروه بیماران متادون 50 درصد مرد و 50 درصد زن بودند. در گروه بیماران که سوفنتانیل دریافت کرده بودند 50درصد مرد و 50درصد زن بودند. میانگین خونریزی در اتاق عمل بر حسب دو گروه متادون و سوفنتانیل معنی دار بود. به نحوی که خونریزی در گروه سوفنتانیل به طور معنی داری بیشتر از گروه متادون بود. میانگین خونریزی در بخش مراقبت های ویژه بر حسب دو گروه متادون و سوفنتانیل معنی دار بود. به نحوی که خونریزی در بخش مراقبت های ویژه در گروه سوفنتانیل به طور معنی داری بیشتر از گروه متادون بود. نتیجه گیری: همانگونه که مشخص گردید میزان خونریزی در اتاق عمل و بخش مراقبت های ویژه در گروه بیماران دارای اعتیاد به اپیوم که سوفنتانیل دریافت کرده بودند به طور معنی داری بیشتر از گروه متادون بود. لذا با توجه به عوارض جانبی کمتر در متادون و لحاظ نمودن سایر موراد به نظر می رسد که گزینه تجویز. متادون گزینه مناسبی باشد.- متادون، سوفنتانیل، خونریزی، جراحی
