Abstract
عنوان مطالعه:تاثیر طب سوزنی در کاهش درد در ریکاوری بیماران تحت عمل جراحی کوله سیستکتومی بازمقدمه:درد، تجربه ای حسی و هیجانی ناخوشایندی است که با آسیب واقعی یا بالقوه بافتی یا ناشی از آن همراه است. بشر همیشه در تلاش بوده است تا راهی برای درمان آلام جسمی و روانی خود بیابد و دردهای خود را تسکین بخشد. در این مطالعه تاثیر طب سوزنی در کاهش درد در بیماران تحت عمل جراحی کوله سیستکتومی بازطی دوره ریکاوری مورد بررسی قرار گرفت.مواد و روشها:این کار آزمایی بالینی دو سو کور بود. به روش نمونه گیری در دسترس، از بین بیماران تحت جراحی کوله سیستکتومی باز تعداد 40 بیمار انتخاب شدند و به طور تصادفی به دو گروه مداخله و کنترل تخصیص یافتند. در گروه مداخله، بعد از اینتوباسیون، و قبل از خروج بیمار از اطاق عمل، طب سوزنی توسط یک نفر متخصص بیهوشی که دوره طب سوزنی را گذرانیده یا آشنا به اصول طب سوزنی و مجاز به انجام این فعالیت بود، در دو نقطه LI4 و LV3 در هر دو طرف مجموعا چهار نقطه بصورت منوآلی پانکچر میشد. و در پایان عمل قبل از اکستوبیشن و به هوش آمدن بیمار سوزنها خارج میشدند. در گروه کنترل نیز قبل از ورود به ریکاوری و زمانی که بیمار هوشیار نبود در همان مناطق پانکچر گروه مداخله فقط چسب زده و بیمار روانه ریکاوری می گردید. نمره درد بیماران توسط تکنسینهای ریکاوری که از نوع مداخله بی اطلاع بودند با مقیاس بصری VAS ارزیابی و در پرسشنامه ثبت میشد. داده ها پس از گردآوری وارد نرم افزار SPSS نسخه 16 شده و مورد تجزیه و تحلیل آماری قرار گرفتند. سطح معنی داری در این مطالعه p کمتر از 05/0 در نظر گرفته شد.یافتهها:میانگین سن شرکت کنندگان در گروه مداخله و کنترل به ترتیب 8/5±8/58 و 49/9±11/57 سال، میانگین شدت درد ریکاوری 45/1±11/5 و 50/1±50/5، القای بیهوشی با پروپوفل 55 و 60 درصد، درصد استفاده از نگهدارنده بولوس 7/66 و 3/33 درصد، و ضریب همبستگی بین سن و نمره درد 138/0 و 235/0 بود. بین گروه مداخله و کنترل از نظر شدت درد و روش القای بیهوشی تفاوت آماری معنیدار مشاهده نشد(P>0.05). اما درصد استفاده از نگهدارنده بولوس در گروه مداخله به طور معنیداری بیشتر از گروه کنترل بود(P=0.027).نتيجهگيري:استفاده از طب سوزنی در بیماران تحت عمل جراحی کوله سیستکتومی باز، تاثیر معنی داری بر کاهش شدت درد آنان در ریکاوری ندارد.کلیدواژهها:طب سوزنی، کوله سیستکتومی، شدت درد- Anesthesiology
