مقایسه اثر دکس مدتومیدین و پروپوفول در کاهش بروز میزان تهوع و استفراغ بعد از عمل در جراحی های لاپاراسکوپیک ژنیکولوژی

چکیده
چکیدهمقدمه: تهوع و استفراغ پس از عمل در 20%-30% بیماران اتفاق میافتد. تهوع و استفراغ دومین عارضه شایع گزارش شدند. ضمن اينكه تجربۀ بسيار ناخوشايندي براي بيمار است و بعضی آنرا بدتر از «درد» توصيف ميكنند .اگر چه ميتواند با عوارض نادر اما جدی از جمله آسپیراسيون محتویات معده، باز شدن بخیه، پارگی مری، آمفيزم زیر جلدی و پنوموتوراكس همراه گردد. سالانه صدها میلیون دلار هزینه PONV میشود. مداخلات فراواني در جهت كنترل PONV انجام گرفته، به دليل عوارض مداخلات دارويي، محدوديتهايي در كاربرد هر يك بوجود آورده است، به عنوان مثال داروی Dropridol به دليل احتمال بروز آريتميهای قلبي در لیست Black Box Warning قرار گرفته است.مواد و روش ها: در اين کار آزمایی بالینی دوسوکور تصادفی شده تعداد 80 بیمار کلاس ASA I,II که کاندید عمل جراحی انتخابی ژنیکولوژیک لاپاروسکوپیک بودند به صورت تصادفی در دو گروه پروپوفول و دکسمدتومیدین قرار گرفتند.داده ها با استفاده از پرسشنامۀ تهیه شده که شامل سن، جنس، وزن و سیگاری بودن و تعیین میزان و شدت استفراغ و زمان بروز تهوع و استفراغ توسط پرستار بخش ریکاوری و نیز پرستار بخش بعد از جراحی زنان تکمیل میشود.این افراد و نیز بیماران نسبت به داروی هیپنوتیک تجویز شده بی اطلاع بودند. ارزیابی شدت استفراغ به وسیله visual analogue scale که معیار 0 تا 10 میباشد در ساعات 2-0 ، 6-2 و 24-6 انجام شد. همچنین داشتن یا نداشتن تهوع نیز ثبت شد.یافته ها: میزان بروز تهوع و استفراغ در گروهی که دکسمدتومیدین دریافت کرده بودند کاهش معنی داری داشت (PV=0.001).نتيجه گيري: نتایج نشان داد كه دکسمدتومیدین علاوه بر اینکه میزان تهوع را کاهش داد شدت استفراغ را نیز نسبت به پروپوفول میتواند بیشتر کاهش دهد (PV=0/001).گل وازگان: دکسمدتومیدین، پروپوفول ،تهوع و استفراغ پس از عمل (PONV) و نمایه بصری استفراغ (VAS)- Anesthesiology
