چکیده
مقدمه: جراحي ستون فقرات يکي از شايعترين اعمال جراحي بزرگ براي بيماران مبتلاء به اختلالات ستون فقرات است. شدت درد پس از عمل جراحي ستون فقرات اصلي متوسط تا شديد است. هدف از اين مطالعه مقايسة طول مدت بيدردي بوپيواکایين و پاراستامول وريدي به تنهايي و به صورت توأم پس از عمل در بيماران تحت جراحيهاي بزرگ ستون فقرات است. مواد و روشها: اين مطالعه به روش کارآزمايي باليني دوسوکور تصادفي در 150 بيمار کانديدای جراحي ستون فقرات انجام شد. بيماران به طور تصادفي به سه گروه 50 نفری تقسيم شدند. در گروه پاراستامول، انفوزيون وريدی 15 میلیگرم / کیلوگرم پاراستامول 20 دقيقه مانده به پايان عمل و ريختن نرمال سالين به عنوان دارونما توسط جراح روی اکسترادورا، گروه بوپيواکايين، ريختن بوپيواکايين با دوز 10 ميلیليتر و غلظت 0.125٪ توسط جراح روی اکسترادورا و انفوزيون 100 ميلیليتر نرمال سالين 20 دقيقه مانده به پايان عمل و گروه پاراستامول و بوپيواکايين، ريختن بوپيواکايين با دوز 10 ميلیليتر و غلظت 0.125٪ توسط جراح روی دورا و انفوزيون وريدی 15 میلیگرم / کیلوگرم پاراستامول 20 دقيقه مانده به پايان عمل شروع شد.پارامترهای هموديناميک، نمرة درد بلافاصله پس از عمل، 30 و 180 دقيقه پس از عمل، مدت آنالژزی، بروز هر نوع عارضه (تهوع، استفراغ، خارش) ، ميزان مصرف مخدر و اولين زمان درخواست آنالژزی ثبت گرديد. يافتهها: تغييرات فشار خون سيستول، دياستول، ميانگين فشار خون شريانی، ضربان قلب و نمرة درد در زمانهای مختلف بين سه گروه تفاوت معنیداری را نشان نداد (05/0<P). در هيچيک از بيماران مورد بررسی عارضهای گزارش نشد. نتيجهگيري: به نظر میرسد انفوزيون 15 میلیگرم / کیلوگرم پاراستامول 20 دقيقه قبل از پايان عمل به همراه ريختن بوپيواکايين با دوز 10 ميلی ليتر و غلظت 0.125٪ توسط جراح روی اکسترادورا در اعمال جراحی بزرگ ستون فقرات موجب افزايش ثبات هموديناميک، افزايش مدت آنالژزی و کاهش بروز عوارض جانبی مربوط به اُپيویيدها میشود.- پاراستامول؛ بوپی واکایین؛ بی دردی؛ همودینامیک؛ اکسترادورا؛ جراحی ستون فقرات
