The Quality of Haemovigilance System after establishment of patient safety standards: The quality of Blood Preparations and Patient Monitoring at the time of transfusion

کیفیت نظام مراقبت از خون پساز استقرار استانداردهاي ایمنی بیمار: کیفیت آمادهسازي خون و پایش بیمار در زمان تزریق

چکیده

زمینه و هدف:

دفتر مدیترانه شرقی سازمان جهانی بهداشت در برنامه بیمارستانهاي دوستدار ایمنی بیمار، خون و فرآوردههاي خونی ایمن را به عنوان یک استاندارد الزامی مورد توجه قرار داده است. هدف از انجام این پژوهش بررسی کیفیت آمادهسازي خون و کیفیت پایش بیمار در زمان تزریق پس از استقرار استانداردهاي ایمنی بیمار بود.

مواد و روش ها:

پژوهش حاضر بر روي یک گروه و در دو نوبت متوالی در بیمارستان بهارلو، شهر تهران انجام گردید. به منظور انجام این مطالعه، داده-هاي مربوط به سال 1391 (اوایل استقرار استانداردها) با سال 1393 (یعنی چهار سال بعد از استقرار استانداردها) مقایسه گردید. تعداد نمونهها 100 عدد کیسه خون بود که توسط فرمول کاکران تخمین زده شده بود. دادهها توسط چک لیست جمعآوري گردید. تحلیل دادهها با استفاده از شاخصهاي آماري توصیفی و آزمون تی زوجی انجام شد.

یافته ها:

7) بعد از استقرار استانداردهاي ایمنی بیمار /96±0/ 6) و پایش بیمار ( 654 /92 ±0/ میانگین شاخص کیفیت آمادهسازي خون ( 299 2) افزایش نشان داد و این افزایش با استفاده از آزمون تی /52 ±0/ 1و 611 /64 ± 0/ نسبت به اوایل استقرار استانداردها (به ترتیب 482 .(P< 0/ زوجی براي هر دو شاخصاز لحاظ آماري معنی دار بود

 

نتیجه گیري:

این پژوهش نشان داد که استقرار استانداردهاي ایمنی بیمار، میتواند بر کیفیت آمادهسازي خون و کیفیت پایش بیمار در زمان تزریق در نظام مراقبت از خون تاثیر چشمگیر داشته باشد.

Abstract
Background and Aim:
The Eastern Mediterranean Regional Office of the World Health Organization has paid attention to
blood and blood products safety as a necessary standard in the Patient Safety Friendly Hospitals
Initiative (PSFHI). The objective of the present study was to investigate the quality of
Haemovigilance system; the quality of blood preparation and patient monitoring at the time of
transfusion, after implementation of the patient safety standards.
Materials and Methods:
This study was done on a single group with a repeated measure design in Baharloo hospital, Tehran,
Iran. The data from 2012 (in the early establishment of standards) was compared with the data from
2014 (i.e. Four years later after implementation of the standards). 100 blood units were selected
through a convenient sampling. The sample size was estimated using the Cochrane ’s Formula. The
data were collected via a checklist. The data was analyzed descriptive statistics and the paired
sample t-test.
Findings:
The mean scores of the quality of blood preparations were (6.92±0.299) and patient monitoring
(7/96±0/654) after the establishment of patient safety standards, The scores increased in comparison
with the early establishment of standards (2.52±0.611, 1.64±0.482). The differences were
significant using a paired t-test (P<0. 01).
Conclusion:
The present study indicated that the establishment of patient safety standards can improve the
quality of blood preparations and patient monitoring at the time of transfusion in the
Heamovigilance system.
