تعیین دقت تشخیص سکته حاد مغزی با معیار FAST توسط کارشناس اورژانس پیش بیمارستانی

چکیده
مقدمه : سکته مغزی از مهمترین علل مرگ و ناتوانی در سراسر جهان بوده و تشخیص و شناسایی زودرس بیماران سکته مغزی حاد توسط اورژانس پیشبیمارستانی میتواند روند درمان بیماران را تسریع نموده، از عوارض بیماری کاسته و در کاهش بار سکته مغزی در جامعه موثر واقع گردد. جهت تشخیص اولیه، از معیار FAST استفاده می شود. هدف از این مطالعه، بررسی دقت تشخیص سکته حاد مغزی با استفاده از معیار FAST توسط اورژانس پیش بیمارستانی، در بیماران منتقل شده به اورژانس میباشد. روش ها: مطالعه حاضر از نوع مقطعی-تحلیلی بر روی 188 بیمار سکته حاد مغزی منتقل شده به اورژانس بیمارستان شهید بهشتی قم در سال 1397 انجام گرفت. دقت و میزان مطابقت تشخیص با استفاده از معیار FAST توسط کارشناسان اورژانس پیش بیمارستانی با تشخیص نورولوژیست تعیین گردید. سطح معناداری 05/0 در نظر گرفته شد. یافته ها: مطابقت تشخیص کارشناسان اورژانس پیش بیمارستانی با استفاده از معیار FAST با تشخیص نورولوژیست 1/76 درصد بود. حساسیت و ویژگی و دقت تشخیصی Face اعلام شده توسط کارشناس به ترتیب 6/96، 2/57 و 60 درصد بود. حساسیت و ویژگی و دقت تشخیصی Arm اعلام شده توسط کارشناس به ترتیب 97، 3/27 و 73 درصد بود. حساسیت و ویژگی و دقت تشخیصی Speech اعلام شده توسط کارشناس به ترتیب 4/97، 58 و70 درصد بود. بحث : دقت تشخیص کارشناسان اورژانس پیش بیمارستانی با معیار FAST در بیماران سکته حاد مغزی نسبتا مناسب بوده و استفاده از این شیوه می تواند روند تشخیص و درمان را در این بیماران تسریع نماید.
