بررسی خطر ابتلا به ایسکمی قلبی بعد از رادیوتراپی پستان و عوامل موثر بر آن در زنان کاندید بیهوشی عمومی جهت اعمال جراحی

چکیده
مقدمه: میزان خطر ابتلا به ایسکمی قلبی پس از رادیوتراپی به صورت دقیق مشخص نشده است و از آن جایی که ایسکمی قلبی نتایج درمان سرطان پستان را با خطر مواجه می سازد بر آن شدیم تا خطر ابتلا به بیماری ایسکمی قلبی بعد از رادیوتراپی پستان و عوامل موثر بر در زنان کاندید بیهوشی عمومی جهت اعمال جراحی به انجام برسانیم. روش کار: در این مطالعه توصیفی مقطعی، 209 بیمار مراجعه کننده به اتاق عمل بیمارستان امام رضا تبریز با سابقه رادیوتراپی به روش سرشماری در یک ساله منتهی به شهریور 1398 وارد مطالعه شدند. چک لیست مشخصات جمعیت شناختی و رز(تشخیص ایسکمی میوکارد) جهت اهداف پژوهش مورد استفاده قرار گرفت و داده ها پس از وارد کردن در نرم افزار SPSS21با آزمون های رگرسیون لوجستیک و ویلکاکسون تحلیل شدند. میزان P کمتر از 0.05 معنادار در نظر گرفته شد. نتایج:انحراف معیار ± میانگین کل نمره شاخص تشخیصی ایسکمی قلبی برابر 0.93±04.88 بود، که 78 بیمار(37.33 درصد) نمره بالاتر از 5 و 131 نفر(62.67 درصد)نمره کمتر از 5را کسب کردند؛هفت عامل بالقوه مرتبط با ایسکمی قلبی در افراد با ریسک بالاپس از رادیوتراپی شناسایی شدند که شامل سن (35/0=OR،009/0=p)، شاخص توده بدنی (38/0=OR،007/0=p)، شیمی درمانی (29/0=OR،005/0=p)، سایز تومور (45/0=OR،005/0=p)، مکان تومور (09/0=OR،02/0=p)، سمت درگیری تومور (33/0=OR،003/0=p) و تعداد جلسات رادیوتراپی (60/0=OR،001/0=p) بودند. نتیجه گیری:رادیوتراپی جهت درمان سرطان پستان به عنوان ریسک فاکتور ابتلا به ایسکمی قلبی در زنان کاندید بیهوشی عمومی برای اعمال جراحی است که فاکتورهایی همچون سن، شاخص توده بدنی، شیمی درمانی، سایز و مکان تومور، سمت درگیر و تعداد جلسات رادیوتراپی تشدید کننده آن هستند.- بیهوشی
