Why Medical Databases Fall Short: A Call for Reform in Medical Registry

Abstract
Reliable, well-structured medical patient databases underpin evidence-based practice, clinical research, and health policy. However, deficiencies in national databases limit the visibility, reproducibility, and clinical translation of local studies. This editorial highlights key structural issues and proposes targeted reforms.
In Iran for a long time efforts have been made to implement medical registries in deferent medical centers. but the results have not been satisfactory due to these databases not being contented with each other. Even in countries with connected patient data bases there is still a lack of certainty about the patients information due to these platforms not being contented to data platforms from other countries. We suggest to establish a worldwide patients' data center to remedy these homecomings.
