مروری بر داروهای مورد استفاده در درمان درد رنال کولیک در مقالات چاپ شده به زبان فارسی؛ یک مطالعه مروری روایتی

چکیده
مقدمه: رنال كوليك(کولیک کلیوی) شايع ترين تظاهر باليني سنگ هاي ادراري و يكي از فوريتهاي شايع اورولوژي است كه به خاطر درد طاقت فرسا براي بيمار بسيار رنج آور است. مداخلات درمانی متفاوتی جهت درد در بیماران مبتلا به رنال کولیک صورت می پذیرد. این مطالعه با هدف بررسی داروهای مورد استفاده در درد بیماران مبتلا به رنال کولیک به صورت یک مطالعه مروری روایتی انجام شد.
روش کار: مطالعه حاضر از نوع مرور روایتی می باشد. در این مطالعه جهت یافتن مقالات مرتبط، پژوهشگران جستجوی کامپیوتری را در پایگاه های اطلاعاتی فارسی زبان SID ، Magiran وScholar Google با کلید واژگان رنال کولیک و کولیک کلیوی انجام گرفت. جستجوی مطالعات بدون هیچ محدودیت زمانی صورت گرفت
نتایج: در مرور پیشینه پژوهش، مطالعات مختلفی بر روی داروهای متفاوتی جهت کنترل درد بیماران مبتلا به رنال کولیک صورت گرفته بود. بیشتر داروهای مورد بررسی، مربوط به دسته داروهای مخدر (مورفین، بوپرنورفین، ترامادول و پتدین) و ضد التهاب های غیر استروئیدی (پاراستامول، دیکلوفناک و کتورولاک) بودند. همچنین داروهایی نظیر کتامین، دسموپرسین، آمینوفیلین و تجویز داخل جلدی آب مقطر نیز مورد بررسی قرار گرفتند.
نتیجه گیری: هرچند استفاده از داروهای مخدر، بهترین اثر را در تسکین درد بیماران مبتلا به رنال کولیک ایجاد می کند، اما ایجاد عوارضی مثل مهار شدن مرکز تنفسی در مدولا و فعال شدن مرکز استفراغ (CTZ) استفاده از مخدرها را محدود کرده است. بدلیل اثرات ضد دردی مشابه با اوپیوئیدها و عوارض کمتر پیشنهاد می شود از دسته داروهای NSAID (کتورولاک) به عنوان خط اول درمان بیماران کولیک کلیوی استفاده شود.
