کیفیت و دامنۀ صدای همنوع در تعامل پزشک ـ بیمار (یک مطالعه کیفی از مشاورههای درمانگاهی)

چکیده
تعامل و گفتگوی پزشک ـ بیمار در مشاوره درمانگاهی، در برگیرنده صداهای متفاوتی است که ضرورتاً همه آنها به مسائل تشخیصی و درمانی مربوط نمیشود. صداهای غیرمرتبط با تشخیص و درمان در مشاورهها، دست کم دو کارکرد مهم دارد که عبارتند از: کمک به تداوم گفتگو و تأثیرگذاری غیرمستقیم در تشخیص و درمان. يكي از صداهای غیرمرتبط که از سوی پزشک مورد استفاده قرار میگیرد، صداي همنوع است که ارجاع دارد به وجه همدلانه تعامل پزشک با بیمار. تحقیق حاضر با رويكرد توصیفی - تحلیلیِ مبتنی بر تحلیل گفتگو، مفاهیم و گزارههای صداي همنوع مورد استفاده پزشکان را در 33 مشاوره درمانگاهي در بیمارستان شهید فقیهی شیراز استخراج کرده است. نتایج تحقیق نشان میدهد که دامنه استفاده از لغات و گزارههاي مرتبط با صداي همنوع 610 مورد بوده كه 3/30% از کل گفتگوها (پزشك و بيمار) و 5/50% از لغات و گزارههاي پزشک را تشکیل میدهد. دامنه استفاده از لغات و گزارههاي صداي همنوع به ترتيب در استفاده از واژههاي تسهيلگر 298 مورد؛ كمك به داستانگويي بيمار 104 مورد؛ استراتژي همدلانه 20 مورد؛ توجه خاص به داستان بيمار 121 مورد؛ و در مورد سوالات نامربوط به سلامت بيمار 6 مورد ميباشد. در رابطه با بستر خاص تحقيق (جامعه ایران) نيز استفاده از مفاهيم خاص 40 مورد و استفاده از مفاهيم ارزشي 21 مورد بوده است. به طور کلی میتوان گفت که اگر چه پزشکان به طور فعال از مفاهيم و گزارههای صداي همنوع در مشاورهها استفاده کردهاند، اما شکل کلی گفتگوها مبتنی بر رابطه نابرابر بین پزشک و بیمار است. از آنجايي كه رابطه نابرابر مانعي در مقابل تعامل است و ارتباط با بيمار نيازمند دانش و مهارت لازم ميباشد، پيشنهاد ميشود نظام آموزش پزشكي در ايران روي آموزش مهارتهاي ارتباطي برنامهريزي كند.
