اثربخشی درمان شناختی مبتنی بر ذهن آگاهی بر کاهش شدت درد و اضطراب مرگ در بیماران دیالیزی: یک مطالعه راهنما

چکیده
زمینه و هدف: بیماران کلیوی نسبت به سایر افراد بیشتر دچار افسردگی بوده و از اضطراب مرگ بالاتری برخوردار هستند. این مطالعه با هدف تعیین اثربخشی درمان شناختی مبتنی بر ذهن آگاهی بر کاهش شدت درد و اضطراب مرگ در بیماران دیالیزی انجام شد.
روش و مواد: این پژوهش از نوع نیمه آزمایشی و در قالب طرح پیش آزمون- پس آزمون با گروه گواه بود. جامعه آماری پژوهش حاضر را تمامی بیماران مراجعه کننده به بخش دیالیز بیمارستان آل جلیل شهرستان آق قلا در سه ماهه اول سال 1398 تشکیل دادند که از بین آنها تعداد 22 نفر، به صورت در دسترس انتخاب و به صورت تصادفی به دو گروه آزمایش و گواه گمارده شدند. ابتدا پیش آزمون برگزار شد. بعد گروه آزمایش به مدت هشت جلسه 90 دقیقه ای تحت درمان شناختی مبتنی بر ذهن آگاهی قرار گرفت و گروه گواه آموزشی دریافت نکرد. سپس پس آزمون برگزار شد. ابزار گردآوری داده ها پرسشنامههای شدت درد و اضطراب مرگ بود. دادهها با استفاده از روش تحلیل کوواریانس تک متغیره استفاده شد و تحلیلهای آماری فوق با بهرهگیری از نرم افزار SPSS-22 به اجرا درآمد. سطح معناداری آزمونها 05/0 در نظر گرفته شد.
یافتهها: میانگین (انحراف معیار) سن گروه درمان شناختی مبتنی بر ذهن آگاهی، (8/6) 9/35 و گروه گواه، (2/6) 8/36 سال بود. در گروه آزمایش میانگین (انحراف معیار) اضطراب مرگ از (5/1) 10 در پیش آزمون به (1/1) 3/6 در پس آزمون کاهش یافت (001/0>P). همچنین در گروه آزمایش میانگین (انحراف معیار) شدت درد از (8/7) 7/31 در پیش آزمون به (5/6) 9/22 در پس آزمون کاهش یافت (001/0>P). میانگین (انحراف) معیار نمرات اضطراب مرگ و شدت درد گروه گواه در پیش آزمون و پس آزمون تفاوتی نداشت.
نتیجهگیری: مطالعه نشان داد که درمان شناختی مبتنی بر ذهن آگاهی منجر به کاهش شدت درد و اضطراب مرگ در بیماران دیالیزی شد.
