وضعیت تکامل کودکان یک ساله در مراجعین به مراکز بهداشتی درمانی شهری جهرم: ارزیابی براساس پرسشنامه سنین و مراحل

چکیده
زمینه و هدف: اختلالات تکاملی و رفتاری یکی از مشکلات شایع در طب کودکان است. غربالگری وضعیت تکاملی به لحاظ تشخیص زودرس این اختلالات جهت مداخله و درمان زودرس از اهمیت زیادی برخوردار است. لذا این مطالعه با هدف تعیین وضعیت تکامل کودکان یک ساله بر اساس پرسشنامه سنین و مراحل (ASQ) در مراکز بهداشتی درمانی شهری جهرم انجام شد.
مواد و روشها: مطالعه حاضر از نوع مقطعی- تحلیلی بود. تعداد 593 کودک یک ساله در مراکز بهداشتی درمانی شهری جهرم سال1394 به روش نمونه گیری طبقه ای تصادفي انتخاب شدند. ابزار جمع آوري اطلاعات پرسشنامه اطلاعات جمعیت شناسی و ASQ، شامل اطلاعات در پنج حیطه بود که توسط والدین کودکان تکمیل شد. اطلاعات از طریق نرم افزار آماری SPSS-21 با استفاده از شاخص های توصیفی و آزمون های Chi-Square و رگرسیون لجستیک مورد تجزیه و تحلیل قرارگرفت.
یافتهها: از بین 593 کودک، 512 (3/86%) نفر تکامل طبیعی داشتند و 80 (4/13%) نفر حداقل در یک حیطه غیرطبیعی بودند. بالاترین اختلال تکاملی در 25 (2/4%) نفر مربوط به حرکات درشت و بعد از آن 19 (2/3%) نفر مربوط به حیطه حل مسئله بود. زایمان زودرس (OR=4.18, 95% CI:1.63-10.7)، وزن کم هنگام تولد (OR=5.17, 95% CI:1.88-14.24)، تغذیه کودک با شیر مصنوعی (OR=2.32, 95% CI:0.89-6.04)، سن بالای مادر (OR=3.06, 95% CI:0.97-9.60) و عفونت دستگاه ادراری (OR=3.52, 95%CI:0.97-12.78) شانس اختلال در حیطه حل مسئله را افزایش دادند. نمایه توده بدنی غیر طبیعی مادر، شانس اختلال در حیطه حل مسئله را 84/2 و حرکات ظریف را 15/3 برابر به طور معنی داری افزایش داد (P<0.05).
نتیجه گیری: مطالعه نشان داد که با وجود تکامل طبیعی در اکثر کودکان، تعداد قابل توجهی دارای اختلال تکاملی حداقل در یک حیطه بودند. بنابراین نیاز به برنامه هایي جهت مقابله با عوامل خطر مرتبط و پيگيري مداوم از نظر تكامل این حیطه ها برای جلوگیری از عوارض در آینده ضروری می باشد.
